Chester Testing: Multi-Practitioner Standardisation Pack

Chester practitioner resources · Standardise

Help different practitioners deliver, record and explain Chester assessments consistently.

This pack provides an SOP template, an observed-practice checklist and a quality-review log. Assign an owner, adapt it to your authorised protocol and approve the local procedure before use. It does not create a new Chester protocol or certify your organisation.

A common process needs a named version.

“We use Chester” is not enough. Record the test, edition, equipment settings, recording intervals, calculation method and applicable organisational standard. Step, cycle and treadmill records are not interchangeable.

1. Local procedure template

Source and editionExact manual, software edition, updates and any organisation-specific instructions.
Suitability pathwayApproved screening process, who resolves concerns, consent and deferral arrangements.
Equipment specificationStep height or ergometer/treadmill capability; device IDs; checks and maintenance responsibilities.
Preparation and briefingParticipant instructions, environmental conditions, familiarisation and standard wording.
Monitoring and stoppingEdition-specific recording intervals, criteria, authority to stop and emergency procedure.
Data and calculationRequired raw observations, units, software/version, quality checks and incomplete-test handling.
Interpretation and communicationReference source, limitations, reporting language and any authorised occupational standard.
Records and accessApproved storage, access, retention, correction process and participant-reference convention.
Quality reviewObserver/reviewer, sample selection, review frequency and action tracking.

Change control

When a protocol, software version or device changes, record what changed, who reviewed it and how practitioners were briefed. Decide whether the change affects comparison with earlier results. Keep the previous version accessible for interpreting historical records.

2. Observed-practice checklist

Practitioner: ____________________ Observer: ____________________ Date: ____________________

Test / protocol edition: ____________________ Observation context: ____________________

For each item record observed / needs attention / not applicable, with evidence. This supports local quality assurance; it is not a substitute for a formal competency assessment.

3. Record review and discrepancy procedure

  1. Select records using your agreed review plan. Include routine work and cases with missing data, unexpected estimates or deviations. Keep the sample and its purpose proportionate.
  2. Trace the result to its inputs. Check participant reference, protocol, settings, stage/time observations, stopping reason, units and software version.
  3. Review anomalies. Check for transcription mistakes, wrong height/workload, signal artefact, mismatched equations or unsupported reference categories. Do not edit values merely to make the trend look better.
  4. Preserve the audit trail. Retain the original record and record any correction, reason, author and date in the approved system.
  5. Resolve before relying on a questionable result. Refer uncertainty to the responsible practitioner. A repeat assessment requires a suitability decision and the correct procedure, not simply a desire for a better number.
  6. Close the loop. Assign actions, review completion and brief the team on recurring issues. Update training or the SOP where the evidence supports it.

Calibration exercise for the team

Give practitioners the same anonymised or fictional raw record and the same authorised calculation method. Compare data entry, calculation and interpretation. Investigate discrepancies against the source record. Do not invent a universal numerical tolerance or call agreement proof of clinical validity.
